Supplements Without a Test: When a Blood Test Helps and When It Does Not
Vitamin D in winter, magnesium for sleep, iron because you are tired, a multivitamin because why not. The usual advice is "get tested first," and it sounds rigorous. It is only half right. A test is worth doing when its result changes what you do; some prevention is recommended to everyone without any test; and for a healthy adult, the bottle taken "just in case" has, for preventing heart disease and cancer, no evidence of benefit for most nutrients, no net benefit for vitamin E, and evidence of harm for beta-carotene. Which of the three you are looking at is the actual question.
When to see a doctor
Right away: a child has swallowed vitamins or iron tablets — call the emergency number or the poison line (112 in the EU and Russia, 103 also for an ambulance in Russia; 911 in the US and Poison Help 1-800-222-1222; 999 in the UK, 111 for advice). Accidental iron overdose is a leading cause of fatal poisoning in children under six (FDA warning on the label). Also right away: sudden confusion, seizures or a person who cannot be woken, whatever they took.
The same day: severe headache, vomiting or unusual drowsiness in someone taking high doses of vitamin A or D.
Routinely: fatigue, hair loss, tingling, brittle nails — these are reasons for an examination, not for a bottle: iron deficiency, for instance, is confirmed by a blood test, and the doctor then looks for the cause, such as an ulcer or heavy periods (NHS, UK). If you are pregnant or planning to be, folic acid and the contents of any supplement are worth discussing early.
What is happening
Three different questions. First: is there a symptom or a risk? Then the answer is an examination and a cause, and a supplement taken beforehand can mask the finding (iron, above). Second: is there prevention that is recommended without a test? Yes — folic acid 400 mcg a day for anyone who could become pregnant, from at least a month before conception (CDC, US); in the UK, vitamin D 10 mcg a day for everyone in autumn and winter, all year for risk groups and for children aged one to four, and 8.5–10 mcg for babies unless they take more than 500 ml of formula a day, which is already fortified (NHS); vegans may need vitamin B12 and iodine from fortified foods or a supplement (NHS). Third: a healthy adult taking something "just in case"? The US Preventive Services Task Force reviewed the evidence in 2022 for one outcome, preventing heart disease and cancer: for multivitamins and single nutrients the evidence is insufficient either way; for vitamin E there is no net benefit; for beta-carotene there is harm — it raises lung cancer risk in smokers. That verdict is about prevention in people without a deficiency; it says nothing against treating a diagnosed one.
"Test before anything" is not the rule either. For vitamin D, the same Task Force found in 2021 that there is not enough evidence that screening symptom-free adults improves anything, and that the measurement itself varies by method. A test earns its place where it changes the decision — iron is the clearest case, because too much of it is a disease too: in hereditary hemochromatosis, most common in people of Northern European descent, iron and vitamin C supplements are to be avoided (CDC, US).
More is not neutral. The UK's upper limits: vitamin D no more than 100 mcg (4,000 IU) a day for adults and from age 11, 50 mcg for children of one to ten, 25 mcg for infants — beyond that, high calcium, weakened bones, kidney and heart damage; vitamin A no more than 1.5 mg a day from food and supplements combined, because years of excess lower bone density, and in pregnancy no liver and no supplements containing vitamin A or fish liver oil (NHS). In the US, the ceiling for preformed vitamin A is 3,000 mcg RAE a day for adults, and for iron 45 mg a day from age 14 and 40 mg below — limits for self-supplementation, not for a treatment dose prescribed and monitored by a doctor (NIH). Supplements are not approved before sale: in the US the manufacturer, not the FDA, is responsible for their safety and labeling.
What to do
- Name the question first. A symptom — examination. A recommended prevention for your situation — take it, no test needed. "Just in case" — the evidence is not there.
- Tell your doctor and pharmacist everything you take, including the "natural" things. Some interact with prescriptions: supplements and medicines on autopilot.
- One supplement, one reason. If you cannot say what a bottle is for, the answer is usually that it is not.
- Add up the doses across products. A multivitamin, a fortified cereal and a separate vitamin D are one intake.
- Iron only on a result. The test and the cause come first: iron deficiency is more common than you think.
- Keep every bottle out of children's reach, iron above all.
When this is not the case
Everything above is about healthy adults. Pregnancy, a diagnosed deficiency, a restricted diet, an illness that blocks absorption, a child — those are situations where a doctor prescribes a nutrient in a dose that may exceed the limits here, and that is treatment, not a hunch.
